It’s an acknowledgement that in order to bring in the new, it is necessary to release the old.

Another symbol appearing here is the Caduceus, the serpent-laden staff that symbolizes healing and the awakening of Kundalini energy. And the fish is a powerful symbol of fertility as well as being immersed in water, symbolic of the intuition.
